Eddie Gluskin is a prominent antagonist within Outlast: Whistleblower.

Right off the bat, Eddie’s a delusional serial killer who, in addition to targeting women, mutilates and murders men in an effort to turn them into women. He cuts people's junk off. We don't have time to unpack all of that, but just know that he's got issues around men and women that look a lot like incel levels of misogyny and misandry.

There's also the setting of the game: Mount Massive, a mental institution. Those who're uncomfortable with institutionalization should be wary.
